Cardiovascular Program

All patients will have their blood pressure screened at all appropriate primary care visits. Patients identified as having high blood pressure will be monitored in our hypertensive program. This is a multidisciplinary clinic that will provide advice on a wide range of topics pertinent to patients with high blood pressure; such as diet, exercise, medication review, blood pressure monitoring and blood work according to the Canadian Hypertension Education Program.

The Registered Nurse will monitor patient’s blood pressure as well as educate about the risks associated with elevated blood pressure. They will recommend targets and educate patients about the cardiovascular risk and how lifestyle factors, as well as personal or family history affects their risk.
