covid-19 self-assessment 
Dear Patients,
 
With the second wave of COVID-19 pandemic and the on-going risks, our appointments  will continue to be virtual visits or telephone appointments. We have access to the OTN video consult platform (eVisit) for this or if you prefer, we can schedule telephone appointments as well. 
 
For any matters that cannot be managed remotely, we will schedule a clinic appointment for you. At this time, we are deferring non-urgent visits (e.g. physical exams etc.) to limit everyone’s exposure and risk. We will renew prescriptions to avoid running low on medications and we will fax lab requisitions to your lab as needed.
If you have respiratory symptoms (cough, fever, difficulty breathing), muscle aches/pains, have travelled outside of the province or country, or have had contact with someone with respiratory symptoms and travel, we ask that you stay home.  Call our office or York Region Public Health COVID -19 Line at 1 877 464 9675  extension 73588 or 1 866 512 6228 for deaf or hard of hearing for further direction. 
Please do not attend our office without instruction in this case.

Aurora-Newmarket Family Health Team (ANFHT)

FHTs are an initiative launched by the Ontario Ministry of Health. The FHT concept involves bringing physicians and other health professionals together to provide high quality primary care.

ANFHT VISION:

The Aurora-Newmarket Family Health Team (ANFHT) will deliver the highest quality, professional patient care that is based on a trusting relationship and is delivered in an evidence based model, is patient centered and provided in a timely fashion.

ANFHT MISSION:

Grow the Aurora-Newmarket Family Health Team (ANFHT) to be supported by a cohesive team of health professionals and administrative professionals. The ANFHT will maintain a focus on effectively and efficiently supporting the changing needs of our patients and the community we service.

ANFHT VALUES:

1. Our environment is a welcoming, patient focused atmosphere that promoted trust, wellness and supportive care. Patients feel valued and respected; and this is enhanced by the health care provider. The patient is involved in their health care decisions.
