The Aurora-Newmarket Family Health Team (ANFHT) will deliver the highest quality, professional patient care that is based on a trusting relationship and is delivered in an evidence based model, is patient centered and provided in a timely fashion.

Grow the Aurora-Newmarket Family Health Team (ANFHT) to be supported by a cohesive team of health professionals and administrative professionals. The ANFHT will maintain a focus on effectively and efficiently supporting the changing needs of our patients and the community we service.

What does this mean for me?

Family Physician

Your family Physician will be your main source for your health care.

Access to other Health Care Professionals

You will have access to other health care professionals within our Family Health Team, such as a Nurse Practitioner, two Registered Nurses, a Registered Dietitian, Occupational Therapist and a Social Worker. If the need is there, we have the right person at the right time!

Urgent Concerns

You will have access to a Family Health Team Physician during extended evening hours for urgent concerns.

If you see another Physician in your Family Health Team for treatment, the EMR chart will be updated regarding the nature of your concern so that he or she will always be up-to date about your medical condition.

High-Quality Primary Care

Our Family Health Team is organized to offer you timely access to high-quality primary care, so you do not have to incur unnecessary visits to Clinics or the Hospital Emergency Room.

Will I get to see my Physician?

Yes. Family Health Teams are a group approach to health care where your Physician will be able to focus on your complex medical concerns but you’ll have access to different health care professionals according to your needs.

What do you expect from me as a patient?

Your appointment time is specially reserved for you, please give us 24 hours notice for all cancellations and contact us to schedule another appointment for you.

While we can appreciate that unexpected circumstances may arise, we reserve the right to charge a missed appointment fee for the service booked should proper notice not be given.

Please always call our office prior to any visit to an outside clinic.

Please understand that if you arrive exceedingly late for your appointment we may be able to offer alternate care, but our admin may have to ask you to reschedule.

We encourage you to arrive 10 minutes early, in order to receive the full treatment and attention you deserve.

Advanced Access

We are using Advanced Access appointment scheduling.

Advanced Access is a set of beliefs, principles and practices that, when implemented, enables a primary care provider to “do today’s work today.”

The core principle of Advanced Access is that patients calling to schedule a provider/physician visit will be offered an appointment the same day for acute care. Our goal is to deal with your concerns as they arise, thus preventing long wait times to see your family Physician.

Those who implement Advanced Access and efficient working methods will find solutions to many of the following challenges:

  • Long wait times for and at appointments
  • Busy waiting rooms
  • No time for providers and team to meet as a team
  • No time for patient self-management
  • High patient use of walk-in clinics and emergency rooms

We continue to pre-book appointments (4-6 weeks in advance) for periodic health reviews, procedures and counselling.

If you are late for an appointment, you will be seen as soon as possible.
